Wall mounting after internal insulation - how?

There are lots of discussions to be had about types of internal insulation, natural vs. synthetic, vapour barriers, and the rest of it. Moving beyond these, if you do internally insulate, you end up with a chuffing gap between your visible wall and your structural wall.
How do you attach anything to it?
Do you have to accept you can no longer have shelves, mirrors, (heavy) pictures on extremal walls, or are there very (and in the case of natural insulations VERY) long screws? And what about curtain rails? What do they get secured to?
Having spoken to some natural insulation manufacturers, I've had a few polite blank stares and no luck with my online search. Any sign posting or explanations welcome.
